Has she been here legally all that time ? If not you will have trouble because she overstayed. There is a guide about petitions for alien relatives under guides at the top.

Did she come to the US with/without visa?
We need more information about her situation.

I think Form I-130 and I-485 are all you need. Do not let her leave the US until she gets a green card. A filed petition means nothing. If she leaves the US before getting the green card, she will be ban from entering for 10 years.

Great advice. She should not leave the US while her petition is being processed.

Your mother will need all the necessary vaccines and a full medical exam done by an approved civil surgeron. He/she will then fill out form I-693 that needs to be filed for the AOS.

Yes, they need to have their medical exams done as well as vaccinations. Go to the USICS Civil Surgeon Locator to find an approved civil surgeon in your area and have the medical exams taken care of. The civil surgeon will fill out form I-693 which will have to be submitted along with the I-130 and I-485.
