I cant get my scanner working so im going to just give a brief of what is being requested...if you have any questions just leave a reply and i will do my best to help!!
First its being called NOA-Supplement to I-129f
3 Questions
1.Did you meet your fiance or spouse through the services of an international marriage broker?
if so provide the name and where the international broker is located.
2.Have you ever been convicted by a court of law <civil or criminal> or court martialed by a military for any of the following crimes.
Domestic violence,sexual assult,child abuse and neglect,dating violence elder abuse and stalking
Homicide,murder,manslaughter,rape,abusivesexual contact,sexual exploration,incest torture etc etc
Three or more convictions for crimes to a controlled substance or alcohol not arising from a single act.
3. If you provided information of any of these listed
was it self acting in self defence
violated a protection order
or I commited,was arrested for, was convicted of of or plead guilty to committing a crime that did not result in serious bodily injury, etc
And thats about it....
It does say that we only have 12 weeks to submitt and if not submitted extensions are not excepted.
Also it will take up to 14 days to process this form.
If you have any of the convicted it is up to you to supply certified copies of all court and police records showing the charges and dispositions for all convictions (This is required even if your recored have been sealed or otherwise cleared or if anyone, including a judge, law enforcement officer or attorney told you that you no longer have a record!!!

congrats Q on being the first to get it and thanks for posting the contents. I find it interesting that there is no mention of the question of whether you have applied for a K1 before - a question that is on the new form but not in the RFE. Therefore we will be not fully compliant with the law once again. Only 3 questions to ask and they forgot one of them. Well done, USCIS!!
One question: Does the form have your name and case number on it or it is just a fill-in form and you write it in yourself?

well at least we can now see why it took them so long to come up with this, its such a substantial piece of work!!!!!
And I am sure that question 1 could be answered by some peoples original petitions, for example myself and my fiance met in person in las vegas while I was on holiday and this was stated in the original petition.
Also what is to stop the petitioner from lying about any possible convictions and if they are going to check up on the information it begs the question why didn't they just check in the first place without holding everything up for months on end

Yes it has a Applicant number/Petitioner A#
Your receipt #
Then a Bar code as well
Blue Paper 3 forms in total
Name =address etc

OR, maybe if they could have made the forms IMBRA compliant on March, instead of June. I think there's a greater chance for pigs to fly than for the USCIS to do things the fast and logical way

It does have a note saying IMBRA imposes limitations on the number of petitions you may file or have approved... if you filed 2 or more in the past you must file a wavier, etc etc etc...

I'm pretty sure only the petitioner needs to sign it or QDilly wouldn't have been able to have mailed it back already.

I think that's where they want to trap people, they want people to lie so they can just smack a big easy denial with hard evidence against them. does anyone else think they are this evil? lol

Qdilly, another question, the stamp from the post office saying what day the RFE was actually posted says what? The same date as the notice or later?

Post office stamp is dated for the 19th and RFE is dated for the 16th
