I have found too much conflicting information about the mailing addresses, so I would like to double-check them again.

For the DS 3032:

For the AOS Fee Bill:

National Visa Center
P.O. Box 790135
St. Louis, MO 63179-0135

Is this correct? Zip codes / P.O. Boxes?

According to the packet I received very recently, these are correct, with one change. The letter I received only had 03801 for the zip code for the DS-3032. It did not have the 2909 on it. However, you should just email the DS-3032 as soon as it's generated anyways instead of postal mailing it back. You'll probably have a response from the NVC days before you even receive the packet. ttyl,
