Hi, we noticed that we need to file the form, with a bit over a month to go! Oops. Anyway, we are still good for time. I am the immigrant, my wife is the United States Citizen.

Could you please help answer a couple of questions? Thanks!

Part 3, Questions 3 asks for the place of marriage. Is this the name of the location, or the City or the State?

Part 4 asks for my spouses "Family Name". Is that her maiden name or her married name (She took my name)?

I honestly thought that my wife's "Family Name" was what her last name is now. So, now I am 50/50. wacko.gif

family name is her married name (unless the person kept their maiden name, then they they would put that)...

there is a place further down the form where it asks for other names used including maiden names..

And since you already stated in your OP that she took your name, then her married name IS her family name.

No there isn't. Look at the section asking about the United States Citizen. It only asks for their family name. There isn't a part that asks for other names used. I don't mean to be argumentative. I hope I have the right version of the form. I downloaded it from the USCIS website. Look at Part 4 "Information about the spouse or parent through whom you gained your conditional residence."

Then again, her last name was the same as mine when we applied for my Adjustment of Status so that makes sense! Thanks.

Your wife's current legal name (married name) is the one to use. If you want you can include a copy of your marriage certificate if it contains her former name.

yes, the family name is her current married name.... which is your last name...
