congrats on your new name! it took a little while for me to get used to it too..
here;s a little something i posted on the old forums when i was researching passport changes
Ok so I am all organized to send off my Passport Amendement paperwork and download the DS-19 document, and find out its all grey color. Ok, it may sound cheap, but they want quality printouts and being that we work from home, ink gets expensive! I decided to call them at 877 487-2778 and spoke to a rep to see if she could send me a form. She indicated the website was incorrect, and the DS-19 is no longer used eff 10/2005 and the website has not been updated. They no longer amend passports.
I mean, I am quite net savy, and being that we have been dealing with immigration for a number of years, I DID DO an additional search to see if there has been any changes not reflected on the Name change page on the DOS website. Either I overlooked it, or its not there. That aside,
The outcome is that being that they no longer amend our passports, we must renew. (I can only ASSume this also will mean we will get the new chip-enhanced version) Fill out the DS-82 renewal form, send in the old passport, marriage certificate, 2 passport style photos, and a check for $67.00 made out to the Dept of State. The new address to send it to is at the bottom of the sheet (Pittsbugh, PA).
