I have no idea how to answer question number 5 of GIV-24 form (Family Composition Sheet).

Question 5 states: "Petitioner: When/how did the petitioner enter US? Type of Visa? A number?"

I entered the states with a visa, and later became a permanent resident, and finally got naturalized. But I have no clue which "A" number they are referring to. I surrendered my green card when I became a citizen and not even sure if I have a copy of it... Is the A number referring to the green card number?

How do U.S.-born citizens answer this question? Thanks.

Yep ! A# is the Green card number!
Best to put it even if naturalized!
