Full Version: Inquiry about DS 156, 156K and 157 Forms
As I have noticed on the sample forms presented and available on JV website on DS 156, DS 156K, DS 157 and some other forms, it seems like on the upper right corner of the page, the form expiration date are displayed. I actually use that form. My question is, does the Date expires which is 7/31/2005 (see sample form on the website) matters a lot? If yes, am I allowed to changed it? I am just thinking it is a government form so any alterations and changes from the original is strictly prohibited unless otherwise there is stipulation. Your response is highly appreciated. Use the DS forms directly from the Dep of States website!
the forms downloaded there expire 2010 for 156 and 156k. ds 157 expires sept 2008. should you have any concerns, dont hesitate to ask..God bless you!

Chris-lei thanks ha, salamat tlaga sa link, buti nlng namalayan ko un kanina, na print ko na tapos pag review ko nakita ko ang expiration date. Baka un pa ang dahilan at magkaproblema kami, thanks tlaga, God bless.
