HELLO VJ MEMBER,
ME AND MY HUSBAND ARE PLANNING TO GO TO PHILIPPINES. BUT I WANT TO CHANGE MY PASSPORT INTO MY HUSBAND SURNAME. MY QUESTION IS, WHAT KIND OF FORM DO I NEED TO FILLED? HOW LONG IT TAKES? AND HOW MUCH IT COST?..PLSS HELP........THANKS IN ADVANCE!

you don't need to change anything. just bring all your original documents (marriage certi., passport/visa...), w-2's, tax return, financial statement.....

I had mine amended in the Philippine Embassy in Washington. I just submitted the marriage certificate plus the application form which can be found in their website http://www.philippineembassy-usa.org/Downloads.htm . The instructions are all the forms. There is no personal appearance unless they require it.
