Medical Exam Procedures

Year 2004


Steps for undertaking medical exams for participants in training programs outside of START.

These steps must be read in conjunction with the Mandatory Reference 253, effective June 18, 2004, Section IX, as well as the Mission Order 253, Participant Training, for regulations regarding health insurance and certification of medical eligibility. These detail HAC (Health and Accident) insurance enrollment, medical eligibility, medical exam validity, and medical waivers.

Participants requiring a medical exam for insurance coverage must have this done at one of USAID/Egypt’s designated medical facilities. To date, there are 3 approved facilities, in Cairo, Alexandria and Assiut (see contact info below). The START contractor is responsible for supplying a Medical Evaluation Voucher to Technical Assistance Contractors for each primary and alternate participant on an approved nomination letter, and for paying for participant medical exams out of FT-800. To access this service, please adhere to the following important notes and steps:

Important Notes:
** Allow for six (6) weeks for complete processing of participants’medical exams and final reports: (a) 1 week for preparation of vouchers, (cool.gif 1 week for participants to schedule and attend appointments, © 2 weeks for medical centers to complete all required tests, (d) 1 week for completion of the center’s medical report and (e) 1 week for START review. Note that if a participant requires follow-up, this may take extra time.
** Once Technical Assistance Contractors receive the final reports, they are to prepare medical waivers for USAID approval, and medical disqualification letters for participants, as needed. Allow for two additional weeks for USAID approval of the waiver, for a total of 8 weeks minimum lead time. Note: Lead time for US programs is 90 days because of visas.
** Vouchers and final reports in hard copy may be picked up from START on Tuesdays and Thursdays from 12 – 3 p.m.
** Advise participants that they are to allow 3 – 4 hours at the medical center, plus possibly an extra appointment if a stress test is required.
** The following is included with the physical exam, and is covered by USAID’s local currency account, FT-800:
(a) laboratory tests for all participants: complete blood count, urinalysis, blood sugar, lipids (cholesterol, including HDL & LDL, triglycerides), creatine (to evaluate kidney function), liver enzymes (ALT & AST);
(cool.gif plain chest x-ray, single view, for all participants;
© the following tests are restricted as indicated: (i) stress test for all males aged 40+, all females aged 45+, and for any other participant based on the physician’s recommendations; (ii) A thallium scan if the stress test is positive; (iii) stress-echocardiogram for selected participants based on the physician’s recommendations; (iv) abdominal ultrasound for selected participants based on the physician’s recommendations; (v) pregnancy test for all married females.
(d) Repeat tests of the above, if necessary, for the doctor to make a final determination about fitness to attend the training
** Treatment for pre-existing conditions, and exams and/or tests required for determination of qualification to travel (not included in the above), will be the sole financial responsibility of the participant.
** Medical disqualifications are not appealable. See the USAID/Egypt Mission Order 253, and Mandatory Reference 253.
** The validity of the medical exam is four months; however, the stress tests and x-rays are valid for 6 months.
** Any dependent approved by USAID for travel, to join a participant in an academic program, must also go through these designated medical facilities, but the costs of such medical examinations and tests will be covered by the participant.
** Note that the exam and all tests will need to be completed with the same medical center.
** Medical exams for interpreter/escorts are not reimbursable by the START contractor.
** Contractors must provide numbers to the START contractor for how many medical exams they will need over the coming year, so that FT-800 levels can be predicted.

Steps:
1. Send a nomination list approved by the CTO to the START contractor, along with the company/project name, address, telephone/fax and contact person responsible for the medicals, one week in advance of the need to distribute vouchers to participants.
2. For tracking purposes, provide a program name, a list of participants indicating which center they will go to, and a program start date.
3. The START contractor will prepare a Medical Evaluation Voucher for each participant, on USAID/START letterhead, with a specific voucher number and indicating which center this will be for. Instructions (fasting, etc.) will be provided.
4. The vouchers will not have a date – TACs should stamp one at the top, with the validity being one week from that. Validity dates help to ensure there will be no large delays on the part of the participant.
5. Participants should call the medical center previously indicated, identify themselves as a trainee funded by USAID and should make an appointment within the one-week validity date of the voucher.
6. Prior to the appointment, participants must complete the three part USAID Medical Examination and Certification Form (AID form 1382-1) available on the USAID web site, and attach their photo on page 2. Participants must completely fill out the medical history on page 1, sections 1 - 14, checking “yes” that they accept the terms, and sign and date the form prior to the appointment. Participants must fill in their names on page 3 which is for USAID’s use. Page 2 is for the doctor’s use. All 3 pages must be present when contractors return this to USAID.
7. Participants should follow the medical exam instructions (fasting, etc.), and bring to the medical exam: (a) the 3-part medical form with photo attached; (cool.gif voucher; © picture ID.
8. Allow for 4 weeks after the initial appointment for all tests to be done, receipt of the reports and review by the START contractor prior to being picked up. The START contractor will call when these are ready.
9. The final report will indicate that the participant: (a) has a medical condition which does not preclude him/her from participating fully in the training program, but which requires a waiver; (cool.gif is medically qualified to travel, with no waiver required; © is medically disqualified, with two physicians signatures on the report indicating concurrence.
10. If a medical waiver is required, contractors who do training outside of START will be responsible for these, filling in the conditions to be waived as written in the medical report sections 26 & 27. If a participant has been disqualified medically, the contractor should prepare a letter to the participant for disqualification.
11. Note: Original X-rays and lab test results will be returned to contractors for distribution to participants; however, they should be retained until the program is over.
12. Vouchers and accompanying invoices will be sent to START by the 3 facilities on a monthly basis and paid for out of FT-800.
