Here are the requirements:

Also is it supposed to be 4.5 cm x 3.5 cm (note this differs from the size and color the US Embassy requires for the visa photos). Make sure the child is looking straight on at the camera rather than their head tilted up or down, etc.
My fiancee had to have the pictures retaken of her child at DFA since her head was tilted down after having had them redone since they were put on white background at a photoshop instead of royal blue. This is new as the old passports used the white background.
Only ID needed for her child was a birth certificate. My fiancee brought her own NBI, birth certificate and passport along.
Be prepared to apply for the passport, then given a date to come back to pay and finish application a week later. Then about a week after that to have the passport ready for pick up.
