Ok we have our infopass next tue for Carlos to get his EAD on day 91..because
we have not gotten it and dont anticipate getting it before tuesday. I know we
need the letters of reciept etc..but do we also need to fill out a new form and
get new pictures and all that?

I've heard that you should bring a newly completed I-765 as well.
When I went to get my interim EAD, they gave me another application to fill out, so to save time, complete and take one with you. All the best, since not all offices are willing to issue interim cards 
File a new application for EAD ....
Then bring along passport, social security card.
Important question: Are they gonna make you pay ANOTHER fee along with the additional I-765?

No fee will be charged if going in for the interim EAD. This will only be valid for approx. 8 months, and the 1-year card will still be sent in the mail.
