Can you please state the text for notarized statement that needs to be provided by US citizen in place of no objection letter for marriage?

Here is the sample, do change it accordingly.
To whomsover it may concern
I, XYZ S/O ZYX, permenant resident of "your address", citizen of United states of America, do hereby solemnly afirm and declare the followings \:-
1) That I was born on "your date of birth" at "place of birth" and have attained the age of maturity.
2) That I have never been married/Or I have been married but I have been seperated and divorced from my spouse for the past ---- years.
3) That I agree to marry "Your fiance's name" D/O "her father's name" of "her address" as per Indian law and fully support her.
4) That I will be held responsible regarding any fault of this wedlock arises in the future. I am giving this confirmation of acceptance of marriage.
Solemnly affirmed and the statement made above are true to the declared before me to the best of my knowledge, belief and information be true by the deponent and I sign on the 19th day of June 2008 who is identified by United states of America.

Note, that the last para is for the notary officer. This is the usual basic, now I don't know if you both are of the same faith or not, it shouldn't matter if you are get married as Civil law, but if you are first doing customary marriage or getting married through Arya samaj, both of you need to be of same religion.
