I am filling out form I751 and I just have a question. In part 3, question 1, where it says--other names used--. do I have to write the name I used in my first marriage or just leave it blank? I'm still using my maiden name so I won't have to put that there. Thank you!!

Yes. If you changed your name to your married name when married, put that there.

Yes, any name that you have used in the past even if you don't use it anymore.
