Hi Everyone,
I have a few questions on filling out form I-134, and hope you can help:
1) Since the form needs to be "sworn to before a notary public," does that mean I need to fill the form out in front of that person? Or can I fill it out in advance and just sign it in front of them?

I feel silly for asking, but I'm not sure I understand how this works!
2) Can I use 401K and Roth IRA investments as "assets?" My income should be sufficient proof that I can support my husband, but just wondering.
3) Question 8 on the form is "The following persons are dependent upon me for support." Do I list my husband there? We don't have any children, so when he enters the US, he'll be my only dependent.
4) Question 10 asks about visa petitions I have already submitted for my husband and the date. Are they looking for the I-130 here, or the I-129F, or both?

As Raymaga said, you have the form filled out & then sign it in front of the notary. Regarding your #2, you don't
have to fill out assets if your income is sufficient, but I can certainly understand you wanting to be certain they approve it! You can fill out any investments on these lines. Just be sure to have a copy of your latest statement as proof just in case they want to see it (they probably won't though.)
Regarding #3, you are supposed to list any existing dependents. i.e.- anybody whom you have claimed as a dependent on your last tax return.
On #4 they want to know if you are sponsoring anyone else too. So list your hubby & put "subject of this affidavit" next to his name.
