Hi everyone, I am having an issue of filling out DS-3032, could you guys help me out from it, please?

Ok, as we could see this on the form as to the following:

I Appoint ____________________ ______________________________
Telephone Number

I am not sure what should I put in right after I Appoint - is it asking the Agent's full name or the Agent's relationship to me?

I had similar question....so I would be putting name of the agent twice on the form.... one after I Appoint ____________________ and one for Name of the person who will act as your agent or attorney for receipt of mail . Is my assumption correct???

I finally decided to email them my ds3032 instead of mailing in the form, so now we have no this problem!!! I recommend you try this too, more faster and less troublesome...
