Does anyone know of the "National identification number" (if applicable) Should this just be left blank?

Just put N/A.
(The people at the embassy will tell you to put N/A or NONE to any left blanks in your DS forms.)

put mine N/A too and if u r are doubtful u can leave it blank too .
