I made some reseach with medexam facilities/costs in Moscow for today, 28 May 2008 and would like to share it with you.
You may probably find it useful.......

IOM
* possibility to fix medexam - even 1 day in advance,
* cost for adult - USD 100 (= RUB 2400 for today's IOM rate),
* cost for child - USD 100 (= RUB 2400 for today's IOM rate),
* certificate cost per person - USD 20 (= RUB 480 for today's IOM rate),
* mode of payment - CASH!

Meditsina
* possibility to fix medexam - 2 weeks in advance,
* cost for adult - USD 100 (= RUB 2356 for today's Central Bank rate),
* cost for child - USD 75 (= RUB 1767 for today's Central Bank rate),
* certificate cost per person = RUB 777 (fixed!),
* mode of payment - both cash and credit cards!

Filatov clinic
* possibility to fix medexam - even 1 day in advance,
* cost for adult = RUB 4000 (fixed!),
* cost for child = RUB 2800 (fixed!), if mantoux test is more than 5 mm, then + additional RUB 600 for x-rays test = RUB 3400,
* certificate cost per person = free of charge,
* mode of payment - CASH!

I chose to go to Meditsina as it is less expensive for me and my kids.
