Can some one please help me out on how to fill out these parts?

Part 6. B "Where have you worked?" - Should I put the head quarter address of my employer, or the address of the office that I worked in? One of my prior employers actually moved after I quit the job. Should I put the old address or their current one?

Part 10, question 8 asking about whether I have joined any organization or association. I was a member of AICPA and a state society of CPA. Should I include these in the answer to question 8? Do I also need to include a separate sheet of paper to explain why I joined these professional associations?

Four of the four last companies I have worked for with all these hostile takeovers, moving to China, etc., don't even exist anymore, so have the same problem with my resume. But I just use the last known address, what else can you use?

You would think Part 10 would be exclusive to such organizations that are anti-American, but that part doesn't state that, had the same question with the number of charitable fund raising organizations my wife belonged to. My attorney said list them, maybe in the future this form will be more specific. I would tend to spell out your acronyms, CPA may be interpreted as the Communist Patriot Association or something like that. Didn't see anything to explain what these organizations were, but did spell them out. AHA is the American Heart Association, and not the Americans for Hitler Association.
