Open the link below which is JSC's and you will find instructions for generating the barcode. Go on "Download the MS Word copy of the DS 230 part I coversheet" and it will open the coverpage containing the address that you should send it to. You have to use that copy provided there as an example of the official DS 230 barcode coversheet and make the required changes on it by following the instructions stated there.
