Be warned, I am most definately NOT an expert on any of this, and I'm very new here, but if you're looking for the affadavit of support form,I think that's the I-864 - at least, that was the one that my wife filed on my behalf in our case.

If you need a copy, you can download it from here as a PDF:

Hope this helps smile.gif

The type of form that needs to completed (as affidavit of financial support) will depend upon what the US embassy/consulate abroad requires. I called USCIS for this very same reason and I was told that it's at the discretion of the embassy. For example I contacted the US embassy where my wife will have her interview, and I was told that they would require the I-134. Good luck you are almost there good.gif .

for K3 it will be the I-134 but please be aware that the affidavit of support must be signed in front of a notary public. You can't just sign it and take it to the interview. It MUST be signed in front of consular officer or notary public

For K-3 Visa interview, it is usually I-134. But the US Citizen Petitioner must sign the I-134 in front of a Notary Public. I am sure, the US Embassy will mail the beneficiary where they will ask you what documents to bring during the interview. You will also need to do a medical prior to the interview.
