Yes, you do send it to Vermont, not the field office. Here are the instructions. Page 5 says to make it payable to U.S. Department of Homeland Security and it specifically says do NOT use initials, you have to actually spell out the words. Good luck! I was so nervous when I put my packet into the envelope...I went over it like four times to make sure everything was signed and there.
