My question is How do I go about my medicals for AOS?He came with the vaccination sheet
from kamoross clinic Lagos,he took 2 dose of the vaccine.Can we send that vaccination sheet with the package?Or do we consult this INS civil doctors?

If the K-3 medical was performed within the last year you do not need a new one, as the old one is in his file and they will use it. Vaccination worksheet is required eventually, either submitted with the application or later when interviewed. (DS-3025 or Vaccination Supplement (I-693A)
