Well I can not find which of the specific "4" g-325a's to put the passport photo. I could only find that it says behind the specific G-325A. Which one is it...

I just don't know.

Stick it behind all of them, I wouldn't separate the four forms, might cause some confusion. Put yours behind your 325 and your SO's behind his/hers. In little ziplock baggies is what most people do, I think.
