I have 2 queries
1) Should the Police certificate obtained from places where the person lived after 16 years of age?
2) DOes the PC has any specific format? What if the PC does not even have a letterhead and handwritten? Would it be a problem during the visa interview?

1. I think that they must be obtained from places where the person lived after 18 years of age.
2. Every location which expedite a PC must have a format where it'll show the seals or something formal. You'll check if for your country is necessary to get PC. Some country don't need it.

Standard is age 16 not 18.
You will not be the first to request this docuement... the agency designated for this request will probably know exactly what is requried.
