I read this from the website, "contact one of the designated physician’s offices to arrange the applicant’s medical exam. Please give the doctor the date of the immigrant visa appointment or explain that the exam is for an immigrant visa applicant. The panel physician will advise you of the cost of the exam and tests". Do i need to contact the physician's office in advance , or just go to st. Lukes with the appointment letter and there are physician's there available to examine??? How much exactly i will pay in peso for the medical exam? Will i pay the medical fee only at St.lukes the day I arrive for the medical examination?

No need to call the clinic. You just go there at St. Lukes. And its better to go when you're not with your monthly period. ($182.65 ) Just bring 8000 pesos and that would be enough with of course change already.

Yes you will pay it the day you plan to have your medical.
