im just confused is i will fill up the ds 157 validated form that the bank gave me....shall i submit all 3 ds 157,the validated one and the two completed forms?

If you have filled out ones, you can just attach the validated blank form from the bank... it's ok. But if you are up to filling out forms again thru handwriting, you can do that too, just make sure you do it neatly and legibly.

Yes, Joyce fill-out the validated ds 157. they will need 2 DS forms 157. the validated DS 157 will also serve as your proof of visa fee payment.
