additional:

250 php for CFO seminar, (the 400) is for the sticker which she could get at DFA right after the seminar
atleaast few copies of your pictures together
no wearing of sleeveless blouse

*and yes I believe there is only 1 location for CFO in Cebu*

and make sure she'll be there atleast 7am in the morning cause they only entertain the first 15 people for a day.

Try to call and book for an appointment. My friend called their office instead of going there so early in the morning and she was included on the next batch's list. Just try if this still works.

CFO cebu is located across USC boys high, also next to 18 cherry court.
Bring proof of relationship such as photos, emails, ym, receipts.. they're gonna ask her how you met, or like when was that picture taken and stuff like that.
