HI VJ MEMBERS!

I NEED YOUR HELP AGAIN. helpsmilie.gif helpsmilie.gif helpsmilie.gif I WAS ABOUT TO FILL-UP THE DS FORMS 156 & 157 BUT THERE ARE SOME QUESTIONS THAT MAKES ME CONFUSED A LITTLE BIT. HERE IT IS..

ON DS 156 QUESTIONS:

#20. NAME AND ADDRESS OF PRESENT EMPLOYER OR SCHOOL NAME:
> AS OF NOW I MANAGE OUR OWN BUSINESS, THAT BUSINESS WAS REGISTERED UNDER MY NAME. ON THE FORM G-325A I PUT THERE THAT MY CURRENT WORK WAS MANAGER/OWNER FOR THE JOB TITLE.

MY QUESTION IS, WHAT WOULD BE THE ANSWER ON THE DS 156? DO I NEED TO PUT MY NAME AS THE EMPLOYER OR I WILL LEAVE IT BLANK?

#21. PRESENT OCCUPATION.
> DO I NEED TO PUT MANAGER/OWNER AS WHAT I HAVE PUT/WRITE ON MY G-325A?

ON DS 157 QUESTION:

#17. LIST ALL EDUCATIONAL INSTITUTIONS YOU ATTEND OR ATTENDED.
>IT IS OK IF I WILL PUT THE "UP" INSTEAD OF UNIVERSITY OF THE PHILIPPINES?

> DO I NEED TO PUT THE TELEPHONE NUMBERS FOR THE SCHOOLS? THE SCHOOL I HAVE ATTENDED DURING MY HIGHSCHOOL I DONT THINK THEY HAVE TELEPHONE NUMBERS.

>AND ALSO IF I NEED TO USE AN EXTRA SHEET FOR MY ANSWERS WHAT IS THE FORMAT? I WILL JUST INDICATE THE NUMBER OF THE FORM LIKE DS 156 AND INDICATE THE NUMBER OF THE QUESTIONS AND THE ANSWER?

Since it is registered under your name, put "Owner" for the job title.

MY QUESTION IS, WHAT WOULD BE THE ANSWER ON THE DS 156? DO I NEED TO PUT MY NAME AS THE EMPLOYER OR I WILL LEAVE IT BLANK?

You are the owner so your name should be there. "YOUR NAME (Owner)"

Put "Manager/Owner"

ON DS 157 QUESTION:

You're not supposed to abbreviate details. If the space is not enough, use an extra sheet as supplement or handwrite it on the field and see if it will fit in.

I couldn't remember putting telephone numbers for all the schools I have attended. I probably put "N/A" on it. But if you can dig it up somewhere or by researching, that would be better.

For every field that you want to use supplemental page, put a notation "See attached supplement" And then on your supplemental page, put the Form number, the question and then the answer. The supplements should be placed after the set of proper forms, not after every page of the form.

Here's an example:

Form G325A, Q: Employment last five years

as for the school names, i think it will fit w/ out being abbreviated since the name of my school is pretty long too but what i did is cutting the the word "university" to just "uni." (Xavier Uni., Ateneo de Cagayan) and it all fit lol... as for the phone #, i didnt include it anymore. i just wrote the street, city and country... good luck and i hope it will go smoothly on you...
