Now i am confused where to put in the information of her address

amori - prefecture
033-0021 - postal code
Japan

I didnt do the same form as you because I just filed the I-130 but this is the format I used for my wife's address. I had to dig up a copy of the form I sent.

Address (Number and Street)
Okamisawa 9-78, Amori
(Town or city)
Misawa
(State/Country)
Japan
(Postal Code)
033-0021

I'm sure others can validate if that's right or not.
I hope this helps.

Hi Smitty.

I hope my advice helpes...
I was in the U.S. for 5 years as an international student before. While I was there and when I applied for student visa before, I turned in all of my forms with my home address in Japan translated as shown below:

Address
1. street number
2. street name
3. (If there is an APT#, put it here)
4. City
5. Prefecture
6. Postal Code
7. Country: Japan

I think you should use the same rule of using commas as a U.S. address.
I never had a problem with this translation( including IRS forms, visa applications(student visa) and so on). We also turned in I-129F and are expecting to hear from them in a couple of weeks.
I wish you luck!!!!
