I'm not sure what you mean by "more sample" -- the affidavit you'll find under Example Forms here is about as good a sample as you will find, but obviously the person who is filling out the affidavit will have to provide his or her own information regarding knowledge of the couple.
The instructions require a sworn affidavit, and there is no question that a notarized affidavit -- using the example affidavit provided here, which indicates that the person took an oath -- is a sworn affidavit. In a pinch, if you cannot find a notary public, federal law allows one to use the following unsworn declaration at the end of the affidavit, which has the same force and effect as notarization: “I declare (or certify, verify, or state) under penalty of perjury that the foregoing is true and correct. Executed on (date). (Signature)”.
