Sorry if someone has already asked this but we have our interview April 23rd and we are very nervous that we might forget something. Can someone give a detail list of what we need for the interview at the Embassy? I called the Embassy today and was told with the new process that started March 19th they dont mail a visa packet anymore just a letter with instructions on how to print the forms on line. Has anyone had an interview scheduled in Manila with this new process and medical also? What or how do we get the medical. All I was told when I called the Embassy was my Fiancee needed to take 6 2x2 photos with white background and current passport and make sure its no later then 1 week before the interview date. Sorry we are just so scared we will not be ready or forget something. Any help from anyone would be great. for Medical appointment, you need to bring:
1. original appointment letter from USEM (then have 1 photocopy)
2. original passport
3. 2pcs 2x2 picture
4. medical fee, pay in peso, SLEC do not accept $ payment
for Interview appointment, you need to bring:
1. proof of visa application payment (the bank's deposit slip)
2. original appointment letter from USEM with SLEC's stamps
3. 2 copies of DS-156 form (glue the 2x2 picture in it), which you downloaded online
4. 1 copy of DS-156k
5. 2 copies of DS-157, validated by the bank
6. original NSO certified birth certificate
7. original NSO certified CENOMAR
8. Affidavit of Support form
attached in the affidavit are: Recent Income Tax Return, Bank Statement, Letter of Employment
9. NBI Clearance, Police Clearance
10. Proof of Relationship (photos, letters, phone bills, engagement ring receipt, etc.)
