Full Version: Philippine Passport Amendment - Filed at the LA Consulate

I have been waiting for my GC and was planning ahead of time to file for the amendment of my passport as I would like to change my single name to my married name. However, I just renewed my passport before I left the Philippines and will not be due for renewal until February 2012. If anyone from here who have experienced undergoing amendment with an unexpired passport filing at the LA consulate please share your experience. smile.gif

REQUIREMENTS FOR AMENDMENT (Philippine Passport)

1.Accomplish consolidated FA Form No. 14, 15 (A).http://www.pcgenla.org/ConFiles/PHILCON_passport_amend.rtf

2.Submit passport for amendment, either in person, or by mail.

3.Processing fee for extension: $25.00; for amendment, $20.00. Plus postage of $6.00, if applicant wishes to have passport to be returned by mail. The Consulate will not be responsible for any loss through mail.

4.Manner of payment: in cash or postal money order, cashier's check or bank draft drawn payable to the Philippine Consulate General, Los Angeles, for the exact amount. NO PERSONAL CHECKS OR PERSONAL MONEY ORDER ACCEPTED.

Since your ammendment is due to change name through marriage you need to report you marriage also to the Philippine Consulate: http://www.pcgenla.org/FaqCivil2con.htm

Thanks so much spidergirl. I already printed the forms and such but still curious if you can do the amendment even if your passport is not expired yet?
