Copy of Homeowners insurance policy with both names
Copy of joint Savings, and 2 seperate checking accounts with both names
Copy of mutual fund with jboth names names
Copy of Citi Mastercard, BJ Wholesale club and Wachovia cards
Two sworn affidavits
Copy of life insurance beneficiary desination form from work
