Question 27 on DS 230 ask for maiden name. if woman didn’t change her name should she write her name of just put None
I would write name there even if it did not change.
Your maiden name is always your maiden name (the name you were born with), so you should put it down.

Full pre-married and previously married name(s). NVC will return the application otherwise.

Yes, Family name before marriage.Hi, YuAndDan, questions again on the Police Report. Does the Police Report need show the timeframe ( from the year when I was 16 years to now ) ? Do you have any idea what the police report looks like? I have not made it and hope can get it done this month end.
Also, can help to explain what the supporting documents together with DS-230/ I-864 my husband need submit? Copy or Original? I get confused.
