Anybody can help me for my little confusion on DS157, i told the guy in BPI that i already had the form filled up thru online. If it is ok for them that they will use it instead of their own form. He said to me that i should attach the form (which validated of payment) in the form i already filled out. IS THIS OK?????
thanks a lot for the answer my fellow vjers..... God Bless!!!

i think most BPI banks only give out validated blank ds157 forms, but some do validate filled in ones too. what i did was fill in the form i got from BPI by hand then include it with all my other forms when i went for my interview earlier. i also used the online form. i read somewhere that the pre-screener will ask you to fill in the blank form if you haven't yet when they check your interview stuffs. goodluck.

yes, all of them b/c it is easier for me. the ds156 form is required by the embassy to be filled in online to get the barcode printed on it. DO NOT sign ds156k until they tell you at the embassy. goodluck.

Some applicants just attached the validated DS-157 with their filled out one while others used the validated one (filled out by hand) and they all made it successfully during their interview. If you will decide to use the form that the bank gave you, make sure you fill it up neatly and legibly (with black ink). That's what I did during my interview.

Good luck!

Just fill-up the validated form from the bank & present with your printed out DS-156 form. They need 2 copies of the form, one validated and the other not. Goodluck!
