Hello all.

My fiancee is awaiting receipt of Packet 3 from Montreal. Does anyone know if those forms (not sure how many) can be completed online? Also do the police report, medical, and I-134 need to go back with Packet 3, or do we just need to bring all those items to the interview? Any assistance with these questions would be much appreciated. Thanks

You fill out the forms they send with the packet 3, and you book your medical and check off all the APPLICABLE items on the checklist they provide, and send back the relevant forms and the checklist. I do believe the form was DS-230 but don't quote me on that, and it was supplied by MTL in packet 3.

The police report has a short lifespan, so get it shortly before you head to your interview. You bring your medical results, the affidavit of support and info, a new letter from the USC stating they are able and willing to marry the beneficiary, the passport photos, long version of the birth certificate, divorce decrees, etc etc as stated in Packet 3's checklist, but you secure the items (or make arrangements to have them before your interview date) and then send back the checklist stating you've got 'em. I think Liz came up with an exhaustive list of what you bring to the interview itself.

If an item on the list doesn't apply to your case, check with the guides to make sure, and then don't worry about it. There are other forms that Packet 4 will ask you to fill out online and bring with you to your interview.

From what I remember Packet 3 was pretty basic ... the forms and the checklist to be sent back. Mind you, I sent my packet 3 in January of last year, so my memory is foggy. Read over Packet 3 when you get it, and then we should be able to clarify things further once you get that far.

you need 2 copies of the DS 156 and DS 156k ( I printed 3 copies , 2 for them and 1 for my files ), wich must be filled out on line , handwriten is not accaptable they will refuess handwritten DS 156 ,
you can also fill out all other forms on line , like DS 230 . and I 134 ( I 134 has to be notorized )
medical as well as police cert are valide for 1 year

Thanks for your input. When the medical is completed, do they give you a sealed envelope to take or forward results directly to the Counsulate?

They send the sealed envelope to your address and you take it with you to the Consulate. The police report has to be within one year of your interview. Leave the medical until as late as possible because you'll need it to be current (within one year) when filing for adjustment of status (AOS) after you're married. (I'm assuming you're a K-1. You should fill in your timeline so we can give the correct answers. unsure.gif )

I guess it depands where you have your medical done ,
( I had mine done in montreal and had to go back 3 days later to pick up the resolts , they gave a brown seald envelope , the X rays to bring to the interview and a copy of the vaccination worksheet for your records keep this worksheet on a save place as you will need it for AOS filling once your in the states)
