I just did mine by mail. I sent the form in on Jan 15th, they issued it Feb 1 and I had it by the 4th or 5th! WAY better than standing in the passport line.

Here is a link to the forms, instructions and addresses: http://www.ppt.gc.ca/index.aspx

As for the name change, I just included a copy of our marriage certificate (I needed both sides signed by my guarantor), I had to include the original of my birth certificate.

Oh, the guarantor rules recently changed, it now can be anyone as long as they hold a valid Canadian Passport - they will need to include their passport number on your application form.

Hope that helps!

It takes about a month or so if you mail it in from the US. You can go in person to a passport office in canada if you want as well.

If mailing:

Send your original proof of canadian citizenship, whether it's a birth certificate or canadian citizenship card. No copies. Send copies of your ID (driver's licence, state id card, perm res card) signed by your guarantor. Watch the pictures. No teeth showing, no smile, no shadows behind your head. On the back it must show which photo studio it was taken at and the date taken. This can be hand written. Be sure to fill in every spot on the form. Be sure to send in your current passport if it's still valid.

Married Name:
Your ID must reflect your new married name in order to have the passport issued in this name.

Any more questions feel free to pm me.

It almost sounds like I work there..... whistling.gif

I was wondering, if you get an EAD stamp in your passport at a POE, would it be wise to wait till you have the two year greencard before surrendering your passport to do a name change? I'm assuming the answer is yes. Is there any problem that may arise from not changing your name on your Canadian passpost right away?

The reason I ask is because I would like to try to get an EAD stamp to try to collect Canadian EI while I wait to get work authorization. So I'm assuming giving up my current passport with the EAD stamp just to change my name, before getting the greencard would not be wise.

Hope this makes sense???? Anybody have any suggestions?

Well if you do any travel by airplane you must book your plane tickets in the name that appears on your passport. If the name on your id is different this may cause you problems as well. Example: Passport is Ms. Maiden Name Driver's Licence is Mrs. Married Name you are essentially 2 different people so would recommend carrying your marriage certificate around.

I took Claudeth to her country's embassy and she got a new passport with her married name. We did have to take all the documentation from the K-1 visa to get it done and also had to have our marriage certificate from the county recorder office.
