im done with my medical last feb 14 and 15 and it was APPROVED.
i was a little bit nervous on my way to SLEC .. i dunno ... maybe it was just anxiety
i was there at 11:20am ..
@ the GUARD i presented the following:
1.) original and photocopy of my appointment letter
2.) original passport
--- they just check it and put stamp on ur original appointment letter then kuya guard who is so nice told me to go to information and give my papers together with my photo
@ the INFORMATION i gave the following:
1.) original and photocopy of my appointment letter
2.) original passport
3.) only 1 photo
--- take a digital photos of me
--- they give me forms to sign in consent for my medical .. it took me 10 minutes to sign it
--- gave it back to them and they will encode it in their computer ... they put stamp on my passport ... **slec ____***
ask some information then after 5 minutes they told me to pay my medical @ the 5th floor
TIP: on the ALIEN NUMBER space ... PUT ur CASE NUMBER .. hehhee coz i forgot mine
5th Floor --- US Counter
1.) Go to information and told me to go to one of the nurse to check my weight and height determinations ..... then i follow a line on the other door for eyesight test ... then to the nurse again to check my vital signs then gave me a number asked me to proceed to Door 7 ... Dr. Jan for my physical exam (so simple ... all the questions asked was on the paper i signed) ... after that they told me to PAY my medical 5, 945 PHP @ the same floor ... it was located on the corner .... at the right side of the elevator ... after that they told me to do my XRAY on the 3rd Floor
--- it took me 1 hr to do all the things on the 4th floor
3rd Floor --- X-ray Dept.
1.) I approach the lady on the table ... she got my papers ... ask me to sign then told me to wait
2.) After 5 minutes .. my turn for xray ... the xray tech told me to INHALE DEEPLY when she said inhale to avoid repeat .. she said exposure to radiation is not good if done several times ... she put a protected gear on my lower back ... it is standard for female ... and IF UR PREGNANT .. u have to DEMAND protected gear for ur front and back ...
--- took me 15 minutes on the 3rd floor ....
4th Floor ---
1.) quick blood extraction and urine test
2nd Floor ---
1.) Immunization interview ... super quick .... no line .. the lady doctor just ask me if i had chicken pox before and i said YES ... she looked for the mark .... coz if u dont have mark ... they will give u chickenpox vaccine .....
2) She also asked for my BCG (bakuna) ... huhuhu i don't have mark ..... but it's okay .... they wont give u another one if she don't see the mark
3.) told me to come back the next day for my vaccination of MMR
4.) Gave the papers to the information and he gave me a piece of paper staple with my receipt ...
stating what time to come back and what to bring on the following day.
5.) He didn't get my passport .... he said i need still it for my vaccination ... and i can go home already coz im done w/ my first day
--- took me 20 minutes on this floor
... i spend almost 2 hrs @ SLEC on my 1st day of medical ... what a relief .. no need for me to wait longer ... IT WAS UNEXPECTED ...
ON MY SECOND DAY ... Feb. 15
-arrived @ 11am .... kuya guard told me to proceed @ the 2nd floor for my MMR vaccination .. approach the same guy @ the information ... he got my receipt .. and after 3 minutes i heard my name .... no line for vaccination again ... then told me to wait at the ground floor for the release of my medical
-wait for 30 minutes before i heard my name .... need to pass 2 photos and passport then wait again ... after 30 minutes my name was called again for the release .... ask me to sign some papers .... gave to me my immunization record and xray after some questions ... told me to go home and they are the one who will forward my passport to USEM ....
--- meet some new friends there ... sophia and jessica ... told them about our website ..... they were happy to know that a site like this exist ....
