and on the bottom right hand corner of the form this information:

My question is this the proper version to use or is there another version? My preocupation is that the form says that it expires 11/30/07.

That's the most current form they have right now. I also used the same form and sent my application on 12/12/07. My check has already been cashed and I've already received my NOA1, so I don't see any problem with using that form.
