I need to request the name change on GC/SSN and stuff but the divorce decree doesn't say my name is changed at all even though I requested it to the court (in Colorado). Does it have to do with me being a foreigner, and had had my married name since I arrived in United States? I thought they'd look up the marriage license record or whatever showing my maiden name... but I guess they just looked SSN up only? The court didn't mention anything about the name change requests I filled it out on several documents of divorce, ended up receiving the decree saying my married name still. I am in a different state and totally lost what to do since I want to change my name back so bad and restore my maiden name in my home country record stuff too... I am sorry my point is scattered around but hope anybody would give me some advice, thanks.
On the FL180 form (Judgment) I have with me, I checked the #4 boxf which says my former name is restored to which I specified before the Judge signed the Dissolution.Although I mentioned it on the initial filing, I made sure on the Judgment that my former name will be restored. Btw, this is in California.I'm not sure if there's still other way to change yours..
