I've just returned from the US for the Christmas holidays only to find out that the Packet 3 was posted to me on the 12th December and never arrived, Iv'e emailed the embassy in London requesting that they resend it and am waiting for a reply to the email which is between 3 and 4 days.
Has this happened to anyone else and if so how long does it take for a replacement packet to be resent?

Thanks.
Jeraly
In the meantime I believe you can download the forms you need online smile.gif

I never received mine the first mailing, either. Someone on this board actually received someone else's! (They sent it on to them with a note.) Anyway, the replacement arrived the week after we contacted the Consulate.

Like Jeraly said, you can easily go to the London Embassy website (http://london.usembassy.gov) and download the forms that will be sent to you in packet 3.

The list of forms that will be sent can be found here. I had all my packet 3 forms ready to go with copies I'd downloaded so that I could get everything sent back the day that the packet arrived.

I'm not sure if things have changed since 2004 but the Embassy wouldn't start processing any of the packet 3 forms until you had confirmed that you had all the documents on the checklist (IV-15 Checklist). It's best to send that completed checklist with packet 3 so everything arrives and is worked on at the same time.

Best of luck.

Thanks for the replies, the forms actually arrived a few minutes after posting the question.
So now I'm happy yet confused.
The DS-156k asks me to attach birth certificate and evidence of support etc. yet the instructions I recieved say not to send anything because I need to bring them with me to the interview.
The DS-156 asks if I intend to work in the US and to give an employers address, of course I intend to work but won't be able to apply for a job until I'm there. So do I fill out this bit or not?

Thanks for the help.
A very confused Dave

Hi there, glad you got your forms ok!
The supporting evidence you need goes with you to the interview. As for the employment bit on the DS-156 I just put that I intended to work, but employer was 'unknown'. Hope this helps!

Thanks for that and good luck with the interview
