Hi everyone! I got the pack3 today and now i am searching for some forms that i could not find.
Does anyone know where i could find the form DS-230 Part I and the DS 157?

I also need to know wheather i have to send all the forms to the embassy or just the DS-230? The instructions say that i have to return the forms: DS-230 PartI, DS-156, DS-157 and DS-156K but i read something else

My daughter is acompanying me i guest i have to return all her forms as well right?

You can find all the forms here: http://travel.state.gov/visa/frvi/forms/forms_1342.html

I think the DS 157 is only for male applicants.

And yes, you would need to fill out the same forms for your daughter as well.

Nice to see things are moving more smoothly now
