It's generally "ok" to make corrections to the document, and I've seen others do it.
However, I'd have him send another, because if they (consular officer) sees that your fiance screwed up something like
middle and last names, he/she might start questioning the validity of the relationship!
My vote is that it's worth the cost to get another document filled out, signed, notarized, and mailed.

If a signature was required on page 1 I would agree. Also, when you print, you need the same paper and printing quality. If it's going to look like the two pages weren't printed together, get a new one. I agree that a need for correcting names on this document gives a bad impression of your bona fides.
