YES ! you have to visit japan consulate here in the philippines to get the police clearance application and to know the exact detail
I don't know about getting one from outside Japan, but my fiance just got his and had to wait to get packet 3 from the embassy and then go to his prefecture police headquarters to order it. He couldn't order it without showing packet 3 (or some form in it).
