What are the forms to be submitted by my sister if she wants to apply for a visitor's visa and where she will submit it?
Form DS-156 and she submits it to the nearest US Consulate or Embassy, fee $100

She also needs to call the embassy to find out what is the procedure to apply for a tourist visa. In Colombia you have to call them to buy a "PIN" that will tell you the date of the appointment.
