Is I-693 needed if you only had 1 MMR shot on your DS-3025?

Dear all,

I am wondering if I should simply send a copy of my DS-3025 along with my AOS packet. The issue is that it only shows *one* MMR shot (which was my first shot). I heard you need two MMR shots displayed on your DS-3025. Will I need to see a civil surgeon to get the missing shot and a completed I-693 form? or can I just send my DS-3025?

Is your Ds-3025 marked in the waiver column with the code for "insufficient time interval"?

If so, you were likely as complete as medically possible on that date and ok for AOS If the rest is marked correctly as well. Try it knowing any adjudicator can impose his interpretation on you. But reading all the rules, you are supposed to be ok with one MMR and the waiver indicated.

Is your Ds-3025 marked in the waiver column with the code for "insufficient time interval"?

If so, you were likely as complete as medically possible on that date and ok for AOS If the rest is marked correctly as well. Try it knowing any adjudicator can impose his interpretation on you. But reading all the rules, you are supposed to be ok with one MMR and the waiver indicated.

Unfortunately, the doctor left the waiver column blank (no A, no B for the MMR...just a blank space). He also left the summary section blank..:(

Knightsbridge clinic does leave the summary section blank but some are still approved despite that. It kinda depends on who's looking at it. But without a waiver marked, it appears you were due a MMR and didn't get it. You most likely will need that shot and get immunizations signed off by a Civil Surgeon.

Thanks greatly for your prompt response. However....It has been a struggle. This morning I called several local civil surgeons, and they have all told me "you need another physical, which will cost around 190 dollars. The surgeon does not transcribe vaccinations written on a record from abroad". I am at wit's end.....

Sorry. That is the downfall of not getting everything signed off at the visa medical. You have to keep calling or look further away to find one. Or threaten to report them to the coordinator of civil surgeons.

Yes they are being out of line. Luckily I ended up calling another doctor who will not rip me off. 50 dollars for the MMR shot and the transcription onto the I-693. Thanks so much for advising me!
