please i want to submit my i-485 for adjustment of status i-765 for employment i-131 for traveling, my question is that do i need to put them together in one envelope or separate, and my wife surname as being change to my surname now, do i need to attache any document for that ?

One envelope would be fine and you should attach a copy of your marriage certificate

October 7, 2015 - NOA2 / our case was approved!!                                                        April 20, 2016 - AOS Delivered!

October 10,2015 - NOA2 Hardcopy Received                                                                 April 26, 2016 - went back to SSA to have my card name changed!

November 12, 2015 - received NVC email that case ready for interview                         April 29, 2016 - received 3 NOAs i-485/i-765/i-131 from the mail! (April 25, 2016 Notice Date)

December 8, 2015 - SLEC Medical but failed on CXR had to do sputum tests!                May 14, 2016 - received mail for Biometrics Appointment Letter (dated May 26, 2016)

December 14,15,16 2015 - went back to SLEC for Sputum Tests!                                   May 26, 2016 - Biometrics done!

December 18, 2015 - Initial sputum tests done! Results are negative                             June 3, 2016 - per case status online i-485 "Ready to schedule for interview"

February 10, 2016 - received email from SLEC sputum tests negative                           July 5, 2016 - 1st SR for EAD (75th day from the received date)

February 17, 2016 - SLEC medical done!!!                                                                       July 16, 2016 - recieved an email EAD case is under review

February 23, 2016 - USEM interview approved! The CO has only 3 questions! LOL        July 22, 2016 - 2nd SR for EAD (92nd day from the received date)

March 4, 2016 - K1 visa packet received!!                                                                      August 2, 2016 - EAD status changed to New Card is being produced

March 8, 2016 - CFO seminar done                                                                               August 3, 2016 - EAD/AP status changed to CASE was APPROVED!

March 19, 2016 - arrived at my POE Los Angeles                                                          August 9, 2016 - EAD/AP combo card received!

thanks aANDg, one more thing .... i have copy of medical examination result with CD that i got from medical center before i went for my k-1 interview in my home country, so do i need to include this to the supporting document ?

Just keep the CD with you.. All you need to attach is the copy of DS3025 vaccination record from the doctor / medical done at your home country.

You need to do medicals here in the Us thats what you need not the kne from nigeria. U have to call uscis and ask for a doctor here to see. They will give you the contact and then call the clinic to schedule.the one you came with from nigeria, take it to the clinic so they know what vaccines to give. They dont need the xray Cd. Trust me

This is incorrect. The K-1 medical is valid. Another medical is not required if you file for AOS within a year of your K-1 medical date. Please read the instructions for the I-485, I-693, and see the pinned topic(plus a ton more of others) all through the AOS forum.

i came in november 2015.i filled in febuary this year for my GC and i got it in april without an interview. And i did medicals here as well. Cos in nigeria i was given just one shot of viracella and also i did TB which wasnt done in nigeria and other blood work.ear,eyes which wasnt done in nigeria. To clear all doubts kindly call uscis and get ur info. Its free. Dont let them send ur application back to u

All the required aspects of the immigrant medical exam should have been done by the consulate appointed panel physician at your K-1 medical exam abroad. The medical is a requirement, so not sure why they would not be giving you the entire exam. > http://www.cdc.gov/immigrantrefugeehealth/exams/ti/panel/technical-instructions-panel-physicians.html

TB is tested by a chest x-ray at a K-1 medical, and they give you a copy of the x-ray on a CD. You are not required to go get another medical, new TB test and more blood work if your K-1 medical was within a year of filing for AOS. You went ahead and did that, but it was not required, and so you should not tell others here that they must get an exam within the US. Their K-1 exam is valid for a year.

If you did not receive all the required vaccinations at your K-1 medical, then you need to see a civil surgeon in the US to get the missing vaccinations and have your vaccination records transcribed onto an I-693. Another medical is not required.

USCIS lists the info in the I-693 form instructions and their policies. You can read them online.

please i need help on this form i-864 part 5, SPONSOR'S HOUSEHOLD SIZE NO 1 AND 3, i dont know what number to write there,i am the principal immigrant as i understand my wife is my sponsoral, so i enter 1 in number 2 YOURSELF with 2 DEPENDANTS in number 4, so now what number should i need to put in 1&3??

#1 is for the immigrant(s) being sponsored in the affidavit of support. #3 would be 0 because the spouse was already counted in #1 as the immigrant. Anything below #1 is for those NOT being sponsored, and as it states on the form and instructions, you do not count anyone twice.

No. Household size is not based on who resides in the same house.
