Lost vaccination records after medical in home country??

Hello!

I just have a question about the adjustment of status process. We already sent in my husband's I-485 package with all of the required documentation and we got the NOA1 on January 20th, 2016. Biometrics appt. is on February 19th. So the process seems to be coming along, but I just realized I overlooked the I-693 and medical information. I didn't send in an I-693 with the transcription from the civil surgeon. When I was filing the I-485, I asked my husband for his vaccination record and he said they didn't give him one after he got ALL of the shots at the medical exam in the Dominican Republic. Now I'm wondering where that vaccination record is - was it inside the Do Not Open package that they gave him when he picked up the results? If so, we turned in that packet at the POE when he came here for the first time. I just assumed that the vaccination record was in there and that USCIS already had what they needed so I didn't need to turn in an I-693, but now I'm reading that we needed to turn it in??

If we do need to do the I-693, how do I get a copy of his vaccination record? Was it inside the packet? Do I need to call the place where he got his medical done originally??

Normally the doctors give a copy of the medical to the applicant so they have it for AOS. They also send one to the embassy. If they didn't give him that, then you'll need to get an appointment with a civil surgeon to get the medical redone which is not cheap. It is a requirement and they issue RFEs for missing medicals or expired ones so it's a big deal. I think USCIS.GOV has the list of civil surgeons. Also depending on if they take his word or not about the vaccines he's received, he might have to redo those as well. The cheapest way to get those done is to go to Walgreens or another pharmacy (or your regular doctor) and have them give the shots to him and then take the documentation to the civil surgeon. I'm sorry you're going through this. It really sucks.

Personally I would wait and see what happens before completing another full medical. There should be a copy in the packet he gave to CBP, so if the vaccination record is complete and the medical has not expired he should be all set. Of course sometimes the complete package from the POE doesn't make it to your local USCIS office, so in that case, they should send you an RFE to let you know if you need another medical.

If you do need to see a civil surgeon, shop around. I visited one where all my wife needed was a Flu shot and a second MMR shot ... then after the doc administered it, I asked him to sign the form and he said he couldn't do that unless she had a full medical. I explained she had one overseas but the DR did not provide for the Flu shot (at the time) or a second dose of MMR .... he said no-go. Wound up paying for another full (and quite unnecessary) medical exam - where all i needed was a simple signature to say she completed the rest of her shots.
