Apostille for Child Birth Certification form DS-4194

Hi Everyone,

Looking for a little help filling out DS-4194 (http://go.usa.gov/3EeXd) on behalf of my new born baby. I need to get her birth certificate apostilled for use in Romania, however, the instructions are somewhat ambiguous as to whether I use mine or my daughter's name in Section 1 or Section 2. My name appears on the birth certificate as the father, so maybe I can leave my daughter's information off of the form all together and simple put my name in section 1?

Hello,

Wrong form! The U.S. State Department in Washington DC does not apostille birth certificates. The birth certificate should be apostilled in the state where the birth was registered. For example, if your child was born in Florida, you need a Florida apostille, if the child was born in California, you will need a California apostille, etc.
