Hey everyone!

I read the post Immunizations. DS-3025 or I-693? pinned on a thread and am still confused. How do we know if my fiancé was given a DS-3025 and what was marked off on it? POE took his sealed medical envelope. We have an appointment tomorrow with a civil surgeon for needed shots, but were wondering if he was given the blank DS-3025 or does the doctor have one to give or is it something we print ourselves and have the civil surgeon fill out. Will he receive another sealed envelope with just the DS-3025 in it to send with AOS? From what i read we don't need a I-693 since his K-1 medical was done overseas, right?

If we don't need the I-693 then can someone explain the following please:

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the Part 7. Vaccination Record completed by a designated civil surgeon. In this case, you must submit Parts 1., 2., 4., and 7. of Form I-693.

So according to this since DS-3025 wasn't included in original submission of medical to POE, then we do need to submit form I-693 again for Parts 1.,2.,4., and 7? what the heck are those and were do we get all these forms??

​Please help me…. Awwww so many questions and not enough time off work to get stuff done…losing my mind.

DS-3025 is completed and issued by the doctor doing the physical for the visa interview, you should have received a copy, (we did) but since you did not, it is 99%certain that it is in the sealed envelope turned over at POE. Why are you going now to a civil surgeon for vaccinations? How do you know what is missing? The civil surgeon will not fill out a DS-3025, he will give you a I-693,without the DS-3025, how can they know what or if vaccinations are required? We would suggest filing the AOS and if you get a REF for the medical, then download or online request from the U.S.C.I.S. form I-693 and then see a civil surgeon who will update it.
